Grades 1-12 Registration Forms:

Registration Requirements:

  1. Proof of Birth for the child (birth certificate -OR- passport: must be an original)
  2. Proof of Guardianship/ Photo ID (state driver's license, state ID, passport, or custody forms)
  3. Proof of Residency (Waltham quarterly tax bill, lease agreement or residency verification form)
  4. Immunization Records, see: Mass Immunization Requirements for the SY 2013-14
  5. Physical Examination (dated within 12 months to the start of school)

If Applicable:

  1. Transcripts or Academic Records (Gr. 6-12)
  2. Disciplinary Reports or 37L (Gr. 9-12)
  3. A current signed IEP
  4. A TB Test is required if the student is coming from an at risk area out of country, see: TB Test Guidelines

*All Minors must be accompanied by a parent or legal guardian. 


English Language Learners (ELL)

Students who indicate another language besides English on their home language survey will be given a language assessment to determine their English proficiency. This information is essential in order for schools to provide meaningful instruction for all students, therefore this assessment must be administered before the child can start school.  Any questions regarding English proficiency testing or ELL Programs should be directed to the ELL department:
