How to Register for Waltham Public Schools?
Students registering for Waltham elementary, middle and high schools (K-12) register at the Parent Information Center, 30 Parmenter Road, Waltham, MA 02453. Please call to schedule a registration appointment at 781-314-5672 or 781-314-5612 (Spanish).

Registration Requirements
- Birth Certificate or Passport (must be an original)
- Proof of Residency (Waltham Quarterly Tax Bill, Lease Agreement or Landlord Letter)
- Mass Transfer Card when transferring from another Massachusetts School. An equivalent transfer form is needed if the student school is out of the state or country.
- Immunization Record & Physical Examination Record (on Massachusetts Dr.'s Letterhead with Dr.'s signature)
- Proof of Guardianship (State Driver's License, State ID or Passport)
- Disciplinary Reports or 37L (Gr. 9-12)
- Transcripts or Academic Records (Gr. 6-12)
- MCAS Tests/Scores (Gr. 6-12)
All Minors must be accompanied by a parent or legal guardian. No registrations will be accepted over the phone or through mail.
